After-hours inquiry triage with clear "not medical advice, call 911" handling
SI sorts overnight messages into queues, flags anything urgent for a person, and uses only one pre-approved acknowledgment.
Admin and operations help for small practices. SI drafts, tracks and organizes; clinicians and staff review and decide. No medical advice or clinical decisions, and patient information stays in tools covered by a BAA.
Worth knowing: Administrative support only: SI does not name a condition, recommend treatment, or make clinical decisions, and does not replace a clinician's judgment. A clinician or trained staff member reviews and approves anything clinical, billing-related or patient-facing. Patient information (PHI) is used only in tools covered by a signed business associate agreement, with access controls and the minimum necessary, never in consumer tools. SI never answers medical questions, assesses symptoms, advises on medication, or tells anyone what to do. Auto-replies are limited to the approved acknowledgment with the 911 line, and SI is not a replacement for the practice's emergency or on-call process.
The problem
Calls and web forms arrive at night and on weekends. Urgent messages get buried, and staff worry about saying too much.
What SI does
SI sorts overnight messages into queues (new patient, existing patient, billing, refill or records, vendor, spam), pulls out name, contact, and request type, flags any message that mentions symptoms or an emergency to the top for a person, and uses one pre-approved acknowledgment: the practice will follow up, the message is not medical advice, and for an emergency call 911.
Examples
The same job looks different in different businesses. These are illustrations, not client results.
- Small family-medicine practice
An overnight form is labeled billing. A second message is placed at the top of the queue for a person. The only automatic note is the approved acknowledgment with the 911 line.
- Solo therapy practice
Weekend messages are sorted into new patient, existing patient, and other. Staff read the queue in the morning and decide who is called back.
- Independent chiropractic clinic
A vendor email is separated from patient messages. Nothing is answered except the one approved note that emergencies go to 911.
What it takes to set up
GemmiSI sets this up on your own equipment and accounts; your data stays with you. Before connecting anything that holds patient information, the practice confirms the tool is covered by a signed BAA, sets role-based access, and limits SI to the minimum necessary. Start with sample or de-identified material. Save the acknowledgment the practice has already approved, the queue names, and what should be flagged for a person. Permission is to read and draft, not to send other replies.
What a human still checks
- A staff member or clinician approves before anything goes to a patient, a payer, or any outside party.
- SI never sends, submits, bills, or posts on its own.
- Anything unusual goes to a "needs clinician or manager review" queue.
- The practice decides what patient data SI may see, and testing starts with synthetic or de-identified samples.
- A person reads the queue first thing and sends every reply other than the approved acknowledgment. The on-call clinician decides on any clinical response.
Try it yourself
Paste this into your SI tool of choice and swap the [bracketed] parts for your own details. It asks SI to stop and show you before it does anything.
Sort these overnight messages [paste] into: new patient, existing patient, billing, refill or records, vendor, spam. Pull out name, contact, and request type. Flag anything that mentions symptoms or an emergency for a person. The only reply you may draft is this approved acknowledgment [paste], which says the practice will follow up, this is not medical advice, and an emergency means call 911. Do not answer medical questions. Pause and show me before sending, submitting, billing, posting or changing anything. Use only the sample or de-identified information I provide; do not include any patient names or health information.
